MMEx enables Advance Care Planning information upload to My Health Record

AHCWA has worked with MMEx to create the functionality for clinicians to upload advance care planning information to My Health Record on behalf of their patients. This includes advance care plans (such as Values and Preferences forms, or other statements of values around end of life care) and Advance Care Directives.

This is a huge step forward, as previously these documents were only able to uploaded by patients themselves.
